It can either be because the author is in hiatus or has abandonned the project. It often means that the story is incomplete. Unfinished: the game is fully fonctional (in opposite to abandonned protypes) but isn't finished.
Games clearly aimed toward a fetish but containing nothing risky will NOT have this label (example: toon characters, cute animations. Not addressed to fulfill a particular fetish but also not really child friendly. More or less SFW : Mainly joke games and similar.
